Introduction
Health Homes is a Medicaid State Plan option that provides an opportunity to build a person-centered system and to expand upon the patient-centered medical home model. Health Home allows individuals to have their health care needs identified and to receive the medical, behavioral, and social supports they need. These needs can be coordinated in a way that recognizes all of the needs as an individual, not just as a patient.

 

Health Home has the capacity to provide all of the following services, as appropriate based on members’ changing needs:

  • Comprehensive Care Management
  • Care Coordination
  • Health Promotion
  • Comprehensive Transitional Care (including appropriate follow-up from inpatient to other settings)
  • Individual and Family Support
  • Referral to Community and Social Support Services
  • Use of Health Information to Link Services

Integrated Care
Research shows that there is a higher rate of psychiatric illness in patients with chronic medical illness, in both children and adults. In the past, it was the tendency of medical providers to work separately, addressing similar illnesses as if they occur in different domains. With integrated care, providers can collaborate on services for the individual and encompass all of their needs.

Integrated care allows us on and Integrated care to focus on and general coordinate healthcare, behavioral health, substance abuse, primary care services, dental services, and other services.

This approach produces the best overall outcomes and aproves the most effective approach to caring for people with multiple health care needs.
